Connecting Medicaid Members to Nutrition Support Through New York State-licensed Social Care Networks (SCNs)
Project FoodBox partners with New York Social Care Networks (SCNs) to make healthy nutrition more accessible for Medicaid members. Providers can refer patients, or members can self-screen, for Health-Related Social Needs (HRSN) services.
How Do You Qualify ?
To qualify, you must demonstrate ongoing need for nutritious, medically tailored food and not currently enrolled under Medicare Advantage Plus (MAP) or Long-Term Care (LTC) plans that already include nutrition support. Our team reviews eligibility to ensure members receive fresh, high-quality produce and pantry items that support your health goals. Project FoodBox makes healthy eating accessible to you and your community.
Enrolled in Medicaid Managed Care
Receive coverage through state-contracted managed care organizations (MCOs). These programs coordinate care, improve quality, control costs, and provide benefits like disease management and care coordination.
State-standardized HRSN Screening
Must have at least one unmet health-related social need (such as food insecurity or housing instability), confirmed using the Health-Related Social Needs (HRSN) Assessment tool issued by your provider.
Meet Enhanced Population Criteria
Eligibility includes people with chronic illnesses (such as diabetes, heart failure, or kidney disease), pregnant moms, older adults, persons with disabilities, or others classified as high-risk by the state.
Additional Clinical or Social Risk Factors
For nutrition support, there must be a documented medical need for dietary help (such as medically tailored meals for chronic conditions or disease management).
